Well, obviously, you would need some skill in your version of MS Word. That cannot be taught in a forum such as this. A google search for medical forms can give you an idea of what you may want. Creating the form is not hard but may be tricky if you are not sure what you are doing. You may want to solicit help from a co-worker with knowledge of Word to assist you.
