What kind of form - userform, or a document with content controls, formfields, or ActiveX controls? Can you attach a document (not just a screenshot) to a post with some representative content (delete anything sensitive)? You do this via the paperclip symbol on the 'Go Advanced' tab at the bottom of this screen.
