Can someone help me with this

I want to create a wizard that will put some details into docs i have created

I want to go into File/New and then the Wizard box appear to choose which ever template I wanted. Once I pressed my template i want the Wizard box to appear asking me what details i want. Once I have entered the details into the wizard it throws the details etc straight into the letter and all you need to do is then print it off as completed.

The details i want are

Name
Address
Private & Confidential
