Personally, I would not recommend using checkboxes. Using checkboxes means you have to implement controls to guard against the user selection two or more of them for the same question. Dropdowns with your three possible choices would be better. Generating the kind of summary text you desire would require a macro. If you need help with this, please attach an actual document with sample questions to a post.
