There are a number of issues, the most significant being that you have apparently set this up as a Letters merge, but the merge document has more than one page. It should have only one page and no Next record field in the last cell of that page.

You have also omitted a header row in your worksheet. Create a header row - call it 'Numbers' and change the merge fields to reflect that.

The attached is a rebuilt version and it should merge correctly when you attach the data source (also attached).
