That org chart most likely manually assembled in Powerpoint and didn't use the automation of smart art. Automated org chart tools have an exclusively hierarchical flow that don't allow the embellishments shown in that sample.

That is not to say you can't create an org chart that looks the same in Word - but you do need to manually draw it using lots of shapes in the drawing palette. Start by going to Insert > Shapes > New Drawing Canvas and then keep adding content from the Insert > Shapes choices. Make use of the elbow connectors for lines and let them snap to the edges of your boxes. It is going to be a slow process until you understand how it is done but it can be built pretty quickly once you get the hang of it.
