Yes. You will want to have the Developer Tab displayed so you can use it.
Each spot you want to type, put a plain text content control. If you will have multiple paragraphs in response to any question, you would want to enable carriage returns in the properties for that control.
Restrict editing to "No changes (Read only)" and mark each of your content controls as an exception.
By going into Design Mode, you can change the hint text in the control if you want. Remember to leave Design Mode when you are done doing this.

You can set a password if you want. I never do if it is just for my use.

You can set your bullets however you would normally do so. You may want to use a table.
