Look into online forms with legacy form fields and look into content controls. I do not recommend mixing the two. Word 2007 does not have a content control checkbox so you may have to use legacy form fields. The last two links below do give you checkboxes in Word 2007 but require macros to implement.

Making Forms with Microsoft Word
Create a Form Using Word Content Controls
Add Classic FormField Controls to Ribbon
Word Content Controls
Content Control Custom Checkboxes
Add Interactive Toogle or Sequenced Object to Document
