Default

The default document name is whatever is in the Title document property. If you were using content controls on your form (and only needed one field to give you the document name) then you could link the name field to the Title document property to get that as your default document name.

If you need to concatenate two separate fields to make the default document name then you will need a macro and could get it to write the values of those fields into the title document property anytime before the user tries to save. It would be best to do this with a content control OnExit macro but that is just one way of doing it.
