I have a word document set up with a picture in the header/footer, with 39 text boxes so people can type their info in each box.

But someone tells me that the intended audience will have a hard time dealing with 39 separate boxes. I agree with this, but I don't know what to do about it.
How do I make it easy for the average, non-savvy user to type their info into all these spaces?
The background image is something I set up in Photoshop, so I can change that to another layout if needed.
Thanks for any help with this.
