One possibility that you might consider now, especially as you are no longer using Word 2010, which is dramatically better with legacy forms than later Word versions, is to take the opportunity to convert the fields to content controls. To aid that, see http://www.gmayor.com/ExtractDataFromForms.htm which includes a process to do this (and another to name the fields to aid data extraction). It also of course extracts data from completed forms to Excel.

I have tested the process on your attached document and it converts the errant fields in Word 2013.
