Default I'm trying to get some fields to add up based on what appears in the field next to each of those

Hi Folks!

Thank you for being here to help people like me!

I am on OS X 10.7.5 and using Excel for Mac 2011 version 14.5.1 (the online version).

I've attached a worksheet with a 'mini-example' of what I'm trying to accomplish with an explanation within the worksheet about which fields need to do what.

Is there a formula that'll do what I need?
