To my knowledge, in Word, you would have to recreate them into forms. To me, it does not appear they were prepared for online/PC forms. They could, however, be converted into fillable .pdf files if you have either a Adobe Acrobat "Professional" version or Standard Version "X" or above. I don't think Word in any version will convert to fillable .pdf files.
Other posters may offer better solutions. |

Well, obviously, you would need some skill in your version of MS Word. That cannot be taught in a forum such as this. A google search for medical forms can give you an idea of what you may want. Creating the form is not hard but may be tricky if you are not sure what you are doing. You may want to solicit help from a co-worker with knowledge of Word to assist you.

Your documents will work just the same in Word 2003-2016 as they did in Word 2002. You refer to them as forms, but all they really are is documents with some tabs & underlining etc. to give the appearance of a form.
If you want the documents to function as forms, you'll need to add content controls or formfields in the relevant locations. If the documents are to be used as forms on Word 2003 & earlier, or on Macs, you're better off sticking with formfields. Read more closely and you'll find neither is required, especially for a simple form.

Is this for use in Word 2002?
Tools for forms (Content Controls) are much simpler in Word 2007 and later. I see your answer to that is "yes." Paul is right, you do not need macros or vba. You do need patience. The simplest way to make answers stay put in forms is to put the form into a Word Table. 4. Use Tables and Tabs to Arrange Text in Microsoft Word To insert form fields, you need to use the Forms Toolbar (View menu). 00 deleteme 1.png To make it work, you will want to protect the document (Tools menu). This is far from simple. Read the articles linked by Paul, or at least skim them. My article is one of those. Making Forms with Microsoft Word That article has been a work in progress for more than a decade and is still not complete. However, Dian Chapman's articles are linked in it under additional resources and may be the best source of information. The links are to archived versions of the articles. |

I charge a lot as a trial lawyer. I would be happy to rewrite your forms for you, at my usual rates. Instead, I am trying to tell you how to learn to do it yourself. To quote: "Well, obviously, you would need some skill in your version of MS Word. That cannot be taught in a forum such as this. You are asking for some kind person to do this work for you for free rather than learning to do it yourself. I am a kind person, but I will not enable your ignorance. I stand willing to help you learn to do this. I spent a lot of time on the article on forms, Dian Chapman spent even more time, I suspect. If you read her articles (even the first two) and then mine, you can do this. If you want a quick reference on things like making forms in Word 2002, consider the book Word X (2002) for Law Firms. |

Again, see if you have a friend who knows Word who can help you. I apologize if I came across as belittling. I am trying to point out that what you want to do takes an investment of time in learning how this stuff works. You are the best judge of whether that investment is worth it for you.

And you're evidently too lazy to do anything for yourself. In case it hadn't occurred to you, forums like this don't exist to do basic work users can do for themselves. Thread closed.
