Hi Everyone,


I am a health professional who is trying to design my own simple forms for me to use during the various parts of a patient's visit (History, examination etc.). What I am trying to do is figure out how I can link text boxes together so that when I type something in one box on one page, it will automatically fill that information out on another page. For example, the patient's name, or date of birth. This information needs to be on every single page for identification purposes, but it's a pain to copy and paste it everywhere that is necessary. There are quite a few other pieces of information that would need to be listed in multiple places as well, so I can't use just the header or footer for this function.

When the document is complete, I will protect it so I can't edit anything in the document except for the fillable text boxes.

Has anybody ever tried to do this? I have been trying to figure it out for a few hours using bookmarks and cross-referencing, but it doesn't seem to be working for me at all. I would greatly appreciate any suggestions!

The link is correct but you do not need to do anything with a userform.
Repeating Data (Populating Multiple Like Fields)
It is likely what you would want are the document property content controls.
