Default Form, add fillable bullets as needed.

Hello,

I am developing a form and need some help. I want to have two rows, the CM and fillable text box, be something that can be copied and pasted or just simply added as needed by the person filling out the form. Thats the only privilege I want them to have. The CM is a list of about 10 different things, someone filling out the form may only need 1 or could use up to all 10.
Is there a way to allow the person filling out the form to be able to control how many of these two rows they need?

1. What Went Well:
  • CM: (This is a drop down list)
  • (Fillable text box)
  • CM: (This is a drop down list)
  • (Fillable text box)

So are there multiple CMs? Or are you just using it as an example that they might use two out of the ten and after each one there's a fillable text box?

Actually not sure, lets assume there could be multiple CM's and there could be CA's or CF's as well.
But if its easier to do with just CM, CA, CF... (10 total) than that could work too.

After each one there is a fill-able text box.
