Default protected form with underline fillable spaces

Hi Friends,

I'm trying to create protected form like :

Address __________ State __________

And for that i used ' text form field ' (no shading) above the drawning lines but when i enter the text in ' address ' , it displaces the text in spaces.

What to do ?
