Default Creating questionaire

Hi guys,

I have two ideas that I would like to implement in my questionaire, but I don't know how to do it or whether it's possible to implement. I hope you could help me:

1. I'd like to create a sliding dot on a bar, something similar as in attached file "1.jpg". The user should be able to move the dot with his mouse along the bar from the beginning to the end. At the end of the bar the value (where the dot is standing) should be displayed. Maybe it's possible to do this by creating userform on VBA, but I don't know how to do that...

2. I know how to create option button where user should be able to select one choice from several possible. However, in my questionaire there are some points where user should make additional choice from two other subchoices if he decides to select for instance bullet 2 (attached file "2.jpg"). BUT, these two additional choices should be disabled if choice 2 isn't selected. That means user has to select choice 2 before selecting any of available subchoices. Does anyone have any idea how to do that?
