Default Previously functional code doesn't work properly anymore

Hello. Does anyone have any idea why previously functional VBA code would stop working properly? I've created user forms and code to customize legal documents with the client's information. I finished a particular automated document back in May. I tested the code (and forms) inside and out and everything worked fine as of a couple of days ago, but now something's gone haywire; after I've put the information in using the forms and run the code, the resulting document is all jumbled. Any thoughts would be greatly appreciated. Thank you!

Without knowing what, exactly, "stop working properly" and "the resulting document is all jumbled" mean, and without seeing either your userform & code or the document/template it applies to, it's impossible to say what the issue might be.

I've once again tested the macro-enabled Word template and (having changed nothing) it now works fine on both my laptop and my desktop; however, it doesn't work properly on my boss' system. I've attached PDFs showing how the first page is supposed to look after the code runs (from my system) and how it looks when it runs on the boss' system.

Your 'code did not work' attachment isn't even of the same file, so it's impossible to make a direct comparison. Furthermore, it's also not apparent that that file was produced by the same code or would do so when running on the same source document that produced the 'code worked' example.
