Question Forms with conditional hidden/visible fields

Hi,

I was wondering if anyone could point me to a resource that would show some examples of how to build conditional formatting into a MS Word document with Form fields.

Specifically what I'd like to do is create a form that allows the user to enter a number in a text field that will then produce that number of a group of fields for them to fill out.

So for example: There would be a group of fields like Author Fname, Author Lname, Address, Phone, Title. The user would fill these out. Then there would be a conditional statement that says "Is there more than one author? If so how many?___ and here they'd put a number. The number would then populate (blank) duplicates of the above for as many authors as are indicated.

Not sure how to start with this.

Alternatively I was thinking, and this may be easier, that if there is more than one author, then it would open an embedded 2nd page or two to fill out (an appended document). Though I'm not sure exactly how to structure that.



I'm trying to keep the initial form simple and no more than 2 pages, but allow for expansion if there's more than one author.

Any help will be appreciated. Thanks!

It would be far better to collect this via a userform. For one thing - if I understand correctly - there would be a need for NEW formfields.

You do NOT want to copy existing formfields. Copied formfields do not have bookmarks, and this severely restricts the ability to get infomation fom them. If you used a userform each iteration of a group would create a new batch of fomfields.

This would also keep the original document file simple.

You could do this for Word 2007 & later, by creating a custom quick part and allowing the users to insert however many of it are required. The custom quick part could contain whatever content controls are required for each record. It would also be possible to drive the quick part insertion via a macro linked to an inputBox that solicits the number required, but that level of complication might not be needed.
