Post Scan Document into excel

I looking for a easy way to scan a document (attached) into excel so I can use this information in word for a mail merge. any help would be appreciative.

I can do the mail merge when the information is saved to excel however instead of writing out a lot of names in excel id rather scan the table into excel as there are over 40 names to input.

Abbyy screenshot reader can do this simply. It has an option to read a table to Excel. However, you didn't attach the document and the image is not of good enough quality to process.
