I'm not sure what you mean about "it brought back 0 in every cell". The formula, which goes in just one cell (not in column B), returns a count (244) of the number of different 'Pt Nbr' records.

I must have typed something wrong. I went back and copied and pasted the formula instead, and I did get 244 now. I'm wondering if I needed to provide the # of admits by month though, how can I have it only count January admits, as an example. instead of the whole year. What would really be helpful is if there was a formula that we could use, (in an admit column) that would show a 1 on the patient, and what day they would have been considered an admit. The reason this would be better is because if we are only showing 244, I can't easily show which patients make up those 244 admits. Since we are going to be doing comparisons between both years, and the different months, chances are there will be times when I have to show which patients are being counted as admits. Would this be possible to have a # populate on the day that we count the admit?

If you want to identify the patients, you could use:
=IF(COUNTIF(B$3:B4,B4)=1,1,"") in Q4 and copy down. That will output a 1 against their first admission. If you want to get the data for a given month, things get more complicated.
